Workplace Harassment Prevention and Response Survey

The primary recommendation is to keep this survey concise, focused and anonymous.

This survey needs to be concise and focused, ensuring it takes no more than 10 minutes to complete. Incorporate true/false, multiple-choice, or open-ended questions, with a particular emphasis on using scales, as they are often the most effective.

The template survey is divided into four sections:

  1. Current Policies: Assesses the existing measures for preventing harassment and violence in the workplace.
  2. Workplace Values and Norms: Highlights the core values and social norms prevalent in the workplace.
  3. Biases: Examines beliefs that may hinder individuals from fully utilizing their talents within the organization.
  4. Personal Questions: Collects data for cross-tabulation to identify if issues are prevalent among specific groups, such as women or individuals with disabilities. This section is not used to track individual responses.
